BRAINS is a multidisciplinary team providing assessment and therapy services to children and adults. We strive to provide the personalized care of a private practice while offering a variety of services typically found at much larger facilities. We are continually evaluating the most current research and client feedback to provide the highest level of care possible.
For physicians or other professionals interested in referring a patient to BRAINS, a referral can be made in a few ways. BRAINS participates with Great Lakes Health Connect to send referrals and share electronic medical records. To make a referral or for more information regarding Great Lakes Health Connect, please visit gl-hc.org . If Great Lakes Health Connect is not available, please fax medical records to 616.365.8971. Patients are also invited to contact BRAINS directly and our staff will answer questions and schedule an appointment with one of our professionals if appropriate. You may also complete the referral form below and our staff will contact the patient. For mutual existing patients, our staff frequently communicates with physicians, teachers, and other professionals to coordinate care according to HIPAA guidelines.

Most services at BRAINS do not require a physician’s referral. Patients may contact the office themselves for most assessment and therapy services. A physician’s script may be required for insurance coverage for physical therapy or occupational therapy. Some therapy services may not be covered by insurance and would be self-pay options only. BRAINS is located in a wooded office park in Grand Rapids, Michigan. We are near 3 Mile Road just off the East Beltline at 3292 N. Evergreen Drive NE. BRAINS Adult & Adolescent Clinic is located at 2900 E. Beltline Ave NE, just a half-mile from our Main Campus. All assessment (testing) clients please report to BRAINS Main Campus. Map and directions
