How to Make a Referral
For physicians or other professionals interested in referring a patient to BRAINS, a referral can be made in a few ways. BRAINS participates with GLHC/Holon/MiHIN (Michigan Health Information Network) to send referrals and share electronic medical records. To make a referral or for more information regarding GLHC/Holon/MiHIN, please visit gl-hc.org .
If GLHC/Holon/MiHIN is not available, please fax medical records to 616-365-8971.
When making a referral, please remember to include the following information about the client:
- Demographic information
- Insurance information
- Desired service/reason for referral
